Who is responsible for your information

The person accountable for privacy at FAM Santé is Hawa D., registered nurse (OIIQ 219 2132), owner of the clinic.

What we collect

We collect only what we need to perform your blood collection and send your samples to the lab.

• Your name, date of birth, phone number and email address.
• Your health insurance number, when the lab requires it.
• The requisition issued by your health professional, along with their name and contact details.
• Clinical details relevant to the collection (for example, a condition to flag before the draw).
• The date, time and type of collections performed.

We never collect your test results: the lab sends those directly to the professional who signed your requisition.

Why we collect it

Every piece of information serves a specific purpose:

• Confirming your identity and validating your requisition.
• Planning, performing and documenting your collection safely.
• Labelling your samples and sending them to the partner lab.
• Reaching you about your appointment.
• Issuing your receipt and meeting our accounting and professional obligations.

We never use your information for advertising, and we sell it to no one.

Your consent

By booking an appointment and coming to the clinic, you consent to our collecting and using your information for the purposes above.

You can withdraw your consent at any time by writing to us. Note that without the essential information we cannot perform the collection safely or have it analysed.

Who we share it with

We share your information only where your care requires it:

• With the partner analysis lab, to process your samples.
• With the health professional who signed your requisition, who receives the results.
• With our booking platform, Medexa, which hosts your contact details and appointment schedule.
• Where the law requires it, or at the request of a professional order during an inspection.

We do not transfer your information outside Canada without first assessing the level of protection it would receive, as Law 25 requires.

How long we keep it

Clinical records are kept for the period required by the Ordre des infirmières et infirmiers du Québec — at minimum five years after the last service provided.

After that period, paper records are shredded and electronic records are destroyed irreversibly.

How we protect it

• Paper records and requisitions are locked in a room with restricted access.
• Electronic records are password-protected and encrypted in transit.
• Only the treating nurse can access your clinical information.
• No health information is exchanged by text message or unsecured email.

If a confidentiality incident creates a risk of serious harm, we will notify you and the Commission d'accès à l'information, as the law requires.

Your rights

Quebec law gives you the right to:

• See the information we hold about you.
• Have inaccurate or incomplete information corrected.
• Withdraw your consent for a given use.
• Ask that your information stop being disseminated, where the law allows.
• File a complaint if you believe your rights were not respected.

Write to us at info@famsante.ca. We answer every request within 30 days at the latest.

This site requires no account and uses no advertising cookies or profiling trackers.

The map on the home page is provided by Google Maps: loading it means your browser contacts Google's servers, which may set their own cookies. Booking happens on the Medexa platform, which applies its own privacy policy.

Changes to this policy

We may update this policy if our practices change. The effective date shown at the top of this page always identifies the current version.
